Trilogy Behavioral Health Outpatient Specialist/Therapist in Chicago, United States

The Outpatient Specialist/Therapist position is a hybrid role. Responsibilities entail providing individual and group counseling to clients with diagnoses ranging from mild to severe mental illness and co-occurring substance use disorders. Responsibilities also include providing case management support to OPS staff and clients as needed, in the form of client advocacy, linkage to services, researching resources, appointment assistance and more. Services will be provided using client- centered, strength based and trauma informed approaches.
Provide comprehensive bio-psychosocial assessment services to clients.
Develop individualized treatment plans with the input of the client, their team, Providers, and natural supports. Collaborate with the client’s team to ensure quality of treatment and coordination of care.
Assess client’s personal, medical, emotional, social, and environmental situation to plan for linkage and treatment course. Serve as an advocate for clients, linking them to needed community services.
Provide individual psychotherapy and skills training to clients from all levels of care.
Coordinate care with primary teams for all other clinical programs/departments.
Provide on-site case management for assigned clients as needed.
Develop and facilitate weekly psychotherapy groups.
Meet with graduate student co-facilitators to ensure effective participation in psychotherapy groups.
Actively participate in consultation opportunities to ensure effective treatment.
Educate and assist clients with applying for entitlements, i.e. Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, and LINK
Accompany clients to important appointments in the community and provide support with issues related to housing, substance use, budgeting, social support, and medication.
Serve as an advocate for clients, linking them to needed community services; assess client’s personal, medical, social, emotional, and environmental situation to plan for linkage and treatment course.
Maintain documentation requirements in compliance with accrediting bodies, funding organizations, and organization standards.
Meet or exceed direct service target for position.
Perform other related duties and/or projects as assigned.
- Must have a) received or be willing to receive the COVID-19 vaccination and booster if due by the date of hire or b) submitted and received approval of a religious or medical exemption, prior to start date, in order to be considered.
Master’s Degree in Social Work, Psychology, Counseling, or other closely related field
Minimum one to two years’ experience working with people with mental health and/or substance use diagnoses.
Clinical experience providing individual and/or group therapy to adults in a community mental health setting preferred.
IM+CANS certification required or must obtain IM+CANS certification within 30 days of hire date.
Valid Illinois Driver’s License with daily access to a vehicle. Must have $100,000/$300,000 auto liability insurance.
Bi-lingual, Spanish speaker preferred
