RiseBoro Homecare Inc. Registered Nurse, Per Diem in BROOKLYN, New York

The Registered Nurse will conduct comprehensive initial and recertification assessments and supervisory visits for a Licensed Home Care Service Agency (LHCSA) and develop a plan of care in accordance with the physicians orders and with the policies and procedures of the agency, the states professional standards and regulatory entities. Additional duties and responsibilities include:

  • Assess clients in their home at start of care to develop a plan of care

  • Initiates Plan-of-Care and makes necessary changes as patient's needs evolve

  • Reassess assigned clients every six months to determine any condition change as well as supervise the home health aide to ensure proper care

  • Perform change in condition visits

  • Supervise/assess Home Health Aides in the client's home every four months, as assigned

  • Educate clients about health maintenance and disease prevention


  • Excellent assessment skill

  • Excellent organizational & time-management skills

  • Basic computer skills; knowledge of data base entry and email (i.e. Outlook)

  • Skilled in interpersonal relationships

  • Excellent documentation skills, ability to meet deadlines

  • Ability to carry out instructions and maintain records

  • Ability to assume delegated authority and accountability for care of assigned patients

  • Ability to function independently with minimal supervision


  • NYS licensed and currently registered as a Registered Professional Nurse
