RiseBoro Homecare Inc. Case Manager, Hope Gardens Senior Center in BROOKLYN, New York

  • Provide comprehensive ongoing case management services including intake assessment, benefit assessment, goal setting, long-term care plan development, information, case assistance, entitlement counseling, referral, supportive contact, advocacy and crisis intervention.

  • Achieve Funding Source Outcomes: Must enroll a minimum of 100 or more clients and assist a minimum of 100 clients with applying for benefits and achieve at least 85% success rate in obtaining services/benefits.

  • Maintain an up to date file on entitlements and benefits

  • Compile case assistance logs, attendance as required by the funding source

  • Maintain client records including case notes, documenting follow-up and outcomes

  • Identify clients who require additional social service services, benefits or entitlements and refer to the appropriate agency for assistance

  • Maintain statistics, prepare reports and other required records in a timely manner

  • Input accurate and complete data for all contacts with clients into the Capricorn database

  • Responsible to enroll clients into funding source required DYCD Connect database

  • Attend relevant trainings in order to stay up to date on resources available to older adults

  • Represent agency in community and interagency activities

  • Adhere to agency policy, procedures and the professional code of ethics

  • Other duties as required by the Director

  • Have a commitment to supporting communities who have experienced systemic oppression and bias (e.g. people of color, LGBTQ+ people, immigrants, justice involved persons, etc.)


  • Bachelor's degree in social work; or

  • Bachelors degree in a related field such as but not limited to counseling, psychology, gerontology with one year of human service experience (including internship and volunteer experience). or

  • Associate's Degree with at least one-year direct experience in case assistance (including internship and volunteer experience); or

  • High School Graduate or equivalent (HSE/GED) with at least one-year direct experience in case assistance and one-year related experience or training (including internship and volunteer experience) in customer service, health and human services
