Turning Points Case Manager in Bradenton, Florida

Position:Case ManagerClassification:Non-Exempt / Full-time

Reports to:Community Service Manager

Position Summary:The Case Manager provides assessment, monitoring, planning, linkage, and advocacy for the most appropriate services to individuals and families with a focus on obtaining or maintaining housing stability.

Essential Duties and Responsibilities:

  • Utilizing a variety of tools and resources, assists clients in progressing toward obtaining or maintaining stable housing.
  • Screens and evaluates applicants for program eligibility, both on the telephone and face-to-face and assists clients in completing all required documentation.
  • Assists clients with the development of housing and service plans to reach and maintain sustainability.
  • Conveys basics of financial management to clients and assists them with development of an individualized budget.
  • Educates clients regarding their rights and responsibilities as participants in a rental assistance program, monitors implementation of policies.
  • Monitors assigned caseload of clients; establishes program plan/goals and evaluates client's progress by conducting routine meetings with client.
  • Provides appropriate and timely follow up to clients, keeping them informed.
  • Effectively links clients with appropriate community resources and advocates for client needs.
  • Maintains accurate, timely, and complete records. Inputs client notes into Community Services Information System (CSIS) within 2 business days.
  • Understands and strictly adheres to each grants eligibility and program requirements.
  • Provides information and documentation necessary for contract compliance in a timely fashion, tracking, maintaining and updating statistics required for reporting.
  • Represents Turning Points at designated community meetings as requested.
  • Works with community agencies to facilitate case management coordination and information sharing.
  • Adhere to agency policy, procedures and the professional code of ethics.
  • Other job-related duties as assigned.

Qualification Requirements:

  • Associate's degree in social work, social sciences, or a related field of study. Significant related work experience in lieu of a credential will be considered. Minimum of 2 years of progressively responsible case management experience. Experience implementing Homelessness Prevention and Rapid Re-Housing Programs (HPRP) preferred.
  • Bi-lingual English/Spanish is a plus.
  • Must have a valid Florida drivers license and reliable transportation.
  • Must be able to complete a pre-employment drug screening in compliance with Florida law.

  • Ability to interview clients and to obtain sensitive information.
  • Ability to effectively and efficiently work on multiple cases at the same time without becoming frustrated or disorganized.
  • Substantial tact and conflict resolution skills to deal with challenges effectively and professionally.
  • Ability to establish and maintain cooperative working relationships with staff and volunteers.
  • Ability to work with culturally and economically diverse individuals in a sensitive, considerate, and appropriate manner.
  • A strong sense of respect for confidentiality involving client , staff, and volunteers.
  • Demonstrated proficiency with Microsoft Office Suite, Google Workplace, and ability to quickly learn other required computer programs and databases.

Supervision:

This position reports to and falls under the general supervision of the Community Service Manager. The Case Manager is expected to use and is given considerable latitude for discretion, independent judgment and self-directed time management in planning and executing work to achieve the most effective results.

Physical Requirements:

This is an office work environment requiring limited physical effort. Position requires visual acuity for reviewing, checking, preparing and maintaining written and computer files. Employees are required to have sufficient hearing to accurately perceive information at normal and spoken word levels.

Mental Demands:

Must handle new and diverse work problems on a daily basis. Personal maturity is an important attribute. Must be able to resolve problems, handle conflict, and make effective decisions under pressure.

Job Type: Full-time

Pay: From $20.00 per hour

Expected hours: 40 per week
