Humana Care Manager, Telephonic Nurse 2 (North Central Region) in Aberdeen, South Dakota


The Care Manager, Telephonic Nurse 2 , in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with partners appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

  • Identifies and resolves barriers that hinder effective care.

  • Makes sure members are identified and communicates with other Medicaid partners for collaborative intervention.

  • Understands department, segment, organizational strategy and operating objectives, including their linkages to related areas.

  • Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.

  • Follows established guidelines/procedures.

Required Qualifications

  • Licensed Registered Nurse (RN) with no disciplinary action (Illinois license is required)

  • 3-5 years of acute care clinical experience

  • Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook

  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences

  • Must have the ability to provide a high-speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required

Preferred Qualifications

  • Education: BSN or Bachelor's degree in a related field

  • Experience with case management, discharge planning and patient education for adult acute care

  • Managed care experience

  • Medicare/Medicaid experience

  • Certified Case Manager (CCM)

  • Hours for this role are: Monday-Friday 8am-4:30pm CST
