The Care Manager, Telephonic Nurse 2 , in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with partners appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

Identifies and resolves barriers that hinder effective care.
Makes sure members are identified and communicates with other Medicaid partners for collaborative intervention.
Understands department, segment, organizational strategy and operating objectives, including their linkages to related areas.
Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.
Follows established guidelines/procedures.
Licensed Registered Nurse (RN) with no disciplinary action (Illinois license is required)
3-5 years of acute care clinical experience
Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Must have the ability to provide a high-speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required
Education: BSN or Bachelor's degree in a related field
Experience with case management, discharge planning and patient education for adult acute care
Managed care experience
Certified Case Manager (CCM)
